To make the registration process quicker and easier, we encourage you to preregister your child. To preregister, call (502) 485-4800 between 8:30 a.m. and 6 p.m.
A parent or legal guardian must accompany the child to the registration desk. Our staff will make copies of your insurance card, photo identification and any needed custody/legal guardianship documents. If you do not bring these legal documents, your child’s surgery may be canceled or delayed until you can obtain the documents.
