How to Apply for WIC

When you visit, please bring the following items:

  • A form of identification (for each person applying): A photo ID, birth certificate or immunization record
  • Proof of address: A current state ID or utility bill
  • Proof of income: The last 30 days of pay stubs or a letter from SNAP, TANF or Section 8 housing
  • Recent height/length, weight and hemoglobin from a medical provider. If you receive medical care from a Norton Healthcare provider, we can access this data for you. If needed, these measurements can be taken at your appointment.

If you or your child already receive care at the Novak Center, future WIC visits can be coordinated alongside your medical appointments. Participants may transfer from any WIC clinic in Kentucky.
